Skilled Nursing

Skilled nursing care is a high level of medical care that must be provided by trained individuals such as registered nurses (RN's) and Licensed Practical Nurses (LPN's).

Premier Point skilled nursing care services can include:

  • Coordinating physical, occupational, and speech therapy, if needed

Therapy

Premier Point's on-staff physical, occupational, and speech therapists will work with your doctor to create a personalized therapy plan that meets your specific needs. Our therapists will discuss your pain, injury, or disability with you, assess your current functionality, and develop a treatment plan aimed at improving, adapting, or restoring any functions that have been impaired or lost. Our goal is to help you achieve measurable improvements in your quality of life.

Premier Point’s physical therapists develop customized care plans based upon the specific condition presented. Plans offer a broad array of therapy techniques including directed and monitored endurance exercises; specific repeated movements to reduce symptoms; strengthening and stabilization exercises; and appropriate education regarding the patient’s role in managing their condition. Premier Point’s occupational therapists are experts in helping patients to regain or utilize adaptive techniques to re-master skills needed for daily living and gainful employment. Therapy provided includes bowel/bladder management, bathing and personal hygiene needs; identifying and learning to use medical equipment and assistive devices; and community mobility and job performance. Our on-staff speech therapists help patients assist patients with feeding and swallowing difficulties and cognitive-communication skills training including attention, memory, and problem solving.

As part of the care plan development process, our therapists work with patients to establish realistic goals, and determine how many therapy visits it may take to reach desired outcomes.



Dietitians

Our registered dietitians use their nutrition expertise to counsel patients undergoing treatment for a variety of medical conditions including diabetes, heart disease, high cholesterol, high blood pressure, and gastrointestinal disorders on how to make unique, positive lifestyle changes that support their medical care plans.

  • Evaluating patient’s food and fluid intake versus their nutritional needs as defined in the care plan and provide recommendations
  • Developing customized dietary programs that support treatment goals including weight management
  • Providing counseling to patient’s family and caregivers to ensure understanding of the critical interaction between diet and treatment goals
  • Reviewing prescribed medications and vitamin supplements for potential dietary conflicts and make recommendations for food restrictions or notify the physician for further review
  • Educating the patient on dietary progress to goals based upon relevant lab results


Medical Social Work

Medical Social Workers help patients and their families with a variety of non-medical needs affecting their well-being. Medical Social Workers are mental health professionals who can provide counseling and referrals for patients if needed.

Medical Social Work services may include:

  • Assistance and advocacy navigating medical and insurance systems
  • Referral to community and disabilities services for assistance with economic and other practical challenges
  • Help applying for senior services such as housing, caregiving, and transportation
  • Mental health services including screenings and supportive counseling
