What is home health care?

Home health care constitutes a variety of skilled, clinical services that can be given in the comfort of your own home, typically after an illness or injury. It is designed to help you recover, regain independence, and become as self-sufficient as possible.

Difference between Home Health & Private Duty?

Certified Home Health is skilled medical care provided by nurses and therapists, ordered by a doctor and typically covered by insurance. Private Duty is non-medical assistance (like bathing, cooking, or cleaning) and is typically an out-of-pocket expense not covered by traditional insurance.

Who is eligible for home health care?

Generally, Medicare covers patients who are under the care of a physician, have a need for skilled nursing or therapy services, and are considered "homebound" (meaning leaving the home is difficult). Commercial insurance requirements may vary, but we can help verify your benefits.

What does "homebound" actually mean?

A patient is considered homebound if leaving home requires a considerable and taxing effort, often requiring the aid of a supportive device (crutches, cane, wheelchair) or another person. Note: Leaving for medical appointments or religious services does not disqualify you!

Is home health care expensive?

Provided a patient meets the requirements, Medicare often covers the full cost of home health services with no out-of-pocket cost to the patient. Commercial insurance plans vary. We always verify coverage and inform you of any potential costs before care begins.

How do I start services?

Your physician must order the services. You can ask your doctor to refer you to Premier Point, or you can give us a call, and we can reach out to your doctor to coordinate the referral for you.

What happens on the first visit?

A nurse or therapist will visit your home to assess your physical condition, review your medications, and discuss your health goals. Together, you will create a personalized plan of care to get you on the path to recovery.
